Scheduling an Appointment

Thank you for your interest in receiving services from the UCMHC!

Due to the nature of our training clinic, we will be closed for the summer semester. We will reopen and be accepting clients in Fall of 2024. We are offering free case management appointments over the phone until April 16th if you are interested in speaking with one of our clinicians about community resources. 

To schedule a free case management appointment, please click the below link. Please note that the first time you click, you may receive an error message. If this happens, please close the tab and try again. Once you reach the site, please click on "UCMHC Schedule Free Phone Case Management", fill out the information, and select a time that works for you.

Please do not fill out any forms other than "UCMHC Schedule Free Phone Case Management" until you hear from us!
